We are looking for a full time PCN Care Co-ordinator to join our friendly and growing team. The role will be based between Longfield Medical Centre and Tollesbury Surgery
We require a proactive candidate who is able to oversee and coordinate with colleagues when needed, by bringing together the different specialist skills of team members to achieve all of the standards required of managing health conditions.
The ideal candidate will have an in-depth knowledge of QOF and IIF, and will ensure that both surgeries are working effectively and efficiently by working collaboratively with various members of the teams to maximise outcomes for both the surgeries and PCN.
The role will require meeting with patients, as well as any relevant family members or carers, to discuss what they need and want from their care, and what services are available to them to support their personalised care requirements, signposting and navigation. They will also work effectively with the clinical team and PCN Social Prescriber to ensure a holistic approach to patient care.
