Registering For Online Access
We can accept online New Patient Registrations through the ‘NHS website – Register with a GP surgery’, however if you require to register for online access to your records, the surgery will require sight of documentation giving proof of identity. Please complete the relevant online application form below and bring the completed forms with proof of ID to reception.
Under 13yrs old can be added to a parent/guardian’s access for appointments and medication.
13yrs – 16yrs no access given for child or parent/guardian.
If requesting full record medical access, which is available for over 16yr old’s, a period of 30 days’ notice is required.
